Year, Make & Model: *		Enter the Year, Make and Model of your vehicle.
Vehicle Identification Number (VIN): *		Enter the complete Vehicle Identification Number (VIN).
Vehicle Size: length x width x height) inches		Enter the overall dimensions of your vehicle. (Length x Width x Height in inches) If you do not have the size, please make sure you specify exact model of your vehicle. We have a large database with all major models and sizes.
Vehicle Weight		Please enter the weight of the vehicle.
Special Instructions:		You may enter Special Instructions. For example: condition of vehicle (non-running), starting procedures, etc.
V. LOADING / DESTINATION INFORMATION

IX. MARINE INSURANCE
All insurance claims must be made within 48 hours of cargo release. Insurance claims must be made to the insurance agent at destination. Please note that there are deductibles on certain cargo or destinations. Please ask your booking office for details. I understand that by waiving marine insurance, Worldwide Shipping Agency or any of its agents or vendors can not be held liable for cargo damages, theft or loss. *

X. PERSONAL EFFECTS OR OTHER SEPARATE ITEMS
No items such as personal effects, parts or other items are allowed inside vehicles unless vehicles are containerized for export and items are listed on a separate packing list provided to us prior shipping. By adding such items I do this on my own risk. I understand that Worldwide Shipping Agency, or any of its agents or vendors can not be held liable for such items and vehicles may be held containing such items till the items are removed. *

XI. DESTINATION CHARGES
Any charges at destination such as port fees, services charges, duties and taxes are additional and are on the shipper / consignee expense. An indication of destination charges can be made upon request by Worldwide Shipping Agency or its agents. *

XII. FREIGHT CHARGES
Prepaid charges must be paid prior cargo being loaded on board of intended vessel. Further, any freight charges not paid at least 3 days prior vessel arrival may delay cargo release which may add additional charges such as demurrage, storage or other to the shipper / consignee expense. Freight charges payable at destination (freight Collect) are subject to collection fees. Please note that local (Florida) checks may take at least 2 days, out of state checks at least 5 days and wire transfer up to 3 weeks (depending on wire transfer type) to clear. *
